Tree of Hope Home Healthcare provides our clients with Homecare services with the highest quality care by our dedicated staff that is trained to deliver Homecare services to the elderly and disabled by assisting with activities of daily living.
Tree of Hope will provide a Direct Caregiver to follow a plan of care based on patient needs.
- Medication reminders
- Meal preparation, and menu planning
- Light housekeeping including vacuuming
- Bathing, and personal hygiene
- Ambulation assistance and fall prevention
- Laundry, shopping and errands
- Assistance with coordination of medical transportation

Upon referral to our agency, we will work closely with family members, hospitals, physicians and other members of the medical team to develop the home service plan that will best meet the needs of the patient. We provide end of life Hospice care and Palliative care. Our team works with your patient’s medical team to provide the needed services, end of life support and assistance that’s recommended/prescribed as needed.
Our Team includes
Registered Nurses and qualified Supervisors to evaluate the patient’s needs, and collectively develop a plan of care based on the patient assessments.
Caregivers, Companions and Home Attendants assist with activities of daily living such as cooking, transportation coordination meal bathing assistance, personal care, dressing assistance, transfers, and taking vital signs.
